I knew I'd seen it somewhere before. It's illustrated in Meshorer's book "TestiMoney".
It's not a coin, it's a lead seal. Or a copy of one, anyway; the style of lettering doesn't match what's illustrated in the book, and the artwork is cruder.
It depicts Jerusalem during the Crusader period, with the Dome of the Rock converted into a church on the left, and the Church of the Holy Sepulchre with it's roof open to the sky on the right.
This copy has presumably been sold as a souvenir in Jerusalem.
